This program is designed for patients without dental benefits and it creates the opportunity for your family and you to have quality dental care in an affordable, convenient way. We believe consistent dental visits not only provide you optimal oral care, but are the best way to keep dental treatment costs down.

Benefits

2 periodic exam per year, every 6 months for adults, teenagers and kids.
1 complimentary exam (emergency or limited exam)
2 regular cleaning per year, in the absence of periodontal disease.
3 oral evaluation for infants per year
All necessary x-ray as prescribed by treating dentist.
20% discount from our Usual, Customary, and Reasonable (UCR) fees for all additional dental services.

Guidelines

Enrollment date begins on the date of enrollment fee is paid and expires after 365 days
Annual enrollment fees are due at the time of enrollment
Valid with health savings accounts, flex spending accounts, and health reimbursement accounts
All membership fees are non-refundable
All additional charges are due day of service
Unused benefits do not roll over after plan expiration date

Exclusions and limitations

Strictly valid only at our office
Not valid with coexisting dental insurances or benefits
Our program is not transferable to another party or uncovered family member
Does not cover any portion of a specialist referral
Not applicable to hospitalization or hospital charges of any kind
Will not apply to dental care for injuries covered under worker’s compensation
Not valid on applicable lab fees, products (electric toothbrush, Waterpik, etc.) or prescriptions.
